  • Agreement upon Reopening Post Covid-19: Working Out Together

    Fitness Incentive has put preventative measures in place, as required or recommended by New York State, to reduce the spread of Covid-19, including monitoring and reducing club capacity, 6ft spacing between equipment and regular, frequent, disinfecting and sanitizing. We would like each member to acknowledge the contagious nature of Covid-19 and agree to work together to protect yourself, our staff, and other members.


    Members must adhere to CDC/State/Local Government Guidelines as well as Fitness Incentive policies outlined below.
    • Every member must also complete a health screening questionnaire and will have your temperature taken each time you visit the gym.
    • Every member will need to complete this Member Liability form to resume working out at Fitness Incentive. Parents of Minor Members will need to complete a Minor Member Liability Form for their underage child.
    • You acknowledge the following member responsibilities:
    • Come Healthy - self-screen before entering the club for any signs of illness, including fever for the past 14 days.
    • Complete the Health Screening Questionnaire (see below) and to inform Fitness Incentive if any of my answers to the Health
    • Screening Questionnaire change. If so, I will seek further screening from a health care professional before returning to the gym.
    • I acknowledge that club capacity is LIMITED, and my admittance is not guaranteed. I may be asked to wait to gain access to workout.
    • I will Check-in when arriving and Check-Out when leaving at the Front Desk so Fitness Incentive can monitor capacity.
    • I will limit my time at the club to 90 minutes per visit per day, maximum.
    • I will maintain Social Distancing- keeping 6 feet apart.
    • I will disinfect all equipment I touch before and after use.
    • I will frequently wash my hands with soap for 20 seconds, or use hand sanitizer when washing isn’t practical.
    • I will wear a protective mask according to CDC and NYS Guidelines.
    • I will comply with the signage in the club.

  • Health Screening Questionnaire

  • If you answered YES to ANY of these questions, further screening is required before you can return to the gym. You should contact your healthcare provider for assessment and testing. An individual who screens positive for COVID-19 symptoms will not be allowed to enter the facility and should contact their healthcare provider for assessment and testing.
If you answer NO to ALL questions, you may participate in gym activities. But you must immediately disclose if and when your responses to any of the aforementioned questions change.

  • By signing this liability waiver, I fully acknowledge the infectious nature of COVID-19 and voluntarily accept the risk of potential exposure to or infection by COVID-19. I understand that the risk of becoming exposed to and/or infected by COVID-19 at Fitness Incentive may result from active or passive negligence of myself or others, including but not limited to, Fitness Incentive employees, its successors or assigns, as well as, its officers, agents, other members and/or other guests. I agree to assume all risks and accept sole responsibility for any injury to myself and/or my child, illness, damage, loss, claim, liability, or expense, of any kind. I further agree to indemnify and hold harmless Fitness Incentive, its employees, and representatives from any and all damages resulting from me and/or my guest entering Fitness Incentive and whether a COVID-19 infection occurs before, during, and after visiting Fitness Incentive.
