Whether or not you’re a supporter of the Affordable Health Care Act (ACA) there’s a feature of the plan that you should be taking advantage of: reimbursement of your heath club membership dues! You read that right—many health insurance providers in New York are now offering money back on your health club dues. We’ve done a little digging—details on the ACA are a tad hard to find—and have compiled the details of the basic benefits offered by the major New York insurance providers. You may be surprised, as we were, to find out that many are offering very generous reimbursements, ranging from $100 to as much as $600! Most require a certain number of visits to the gym to qualify, typically 50 in a 6 month period. That’s twice a week. Most FI members cover that with ease. Moreover, our new online account services make it a snap to download your attendance records so providing proof of membership and attendance is a snap.
You should definitely check (see below for basic info and links) to see if your insurance carrier participates, and contact them for the specifics of the coverage that applies to you. Otherwise, you’re probably leaving money on the table. And let your friends know, too. Don’t let them settle for a cheap, inferior gym experience when they could be enjoying Fitness Incentive for a similar cost. Friends don’t let friends join crummy gyms, right?

You or your family can get up to $200 per subscriber contract, per calendar year for membership dues at a fitness center. You just need to exercise regularly. Because plans vary, you will want to make sure you’re eligible for
this fitness reimbursement program. Just call the Customer Service number on the back of your ID card.
To qualify for your reimbursement, you must meet the minimum exercise requirements: Exercise at your fitness center 48 times within the calendar year. Each time you exercise, record it on your Fitness Reimbursement Program Log Card and ask a fitness center staff person to initial it, or
get a copy of your fitness center’s computer printout.
1. Pay your fitness center dues and keep the receipt(s). Receipts must be original and include the name of the fitness center, description of the membership purchased, date of payment, amount paid and the
name of the person using the membership. If your fitness center dues are electronically debited from your bank account, ask for a receipt or submit
copies of your bank statements with the specific withdrawals circled.
2. Start your exercise program. Use the Fitness Reimbursement Program Log Card to record each time you exercise and ask a fitness center staff
member to initial it. Or, use your fitness center’s computer print out.
3. Complete the Fitness Reimbursement Form.

PPO plan: Every six months, employees and their covered dependents are eligible to receive reimbursements of gym membership fees up to $200 and $100 respectively. Needs 50 visits in 6-months.
$200 for 50 visits in a six-month period, and an additional $200 for 50 visits in a second six-month period, for a total of $400 for the entire plan year. Spouse can receive $100 for 50 visits in a six-month period, and an additional $100 for 50 visits for the second six-month period, for a total of $200 for the entire plan year.
You may be reimbursed up to $200 on gym membership fees if you go to the gym at least 50 times within a consecutive six-month period. In addition, if your spouse or domestic partner is also actively enrolled and covered under your Health Republic plan, then he or she may be reimbursed up to $100 for going to the gym at least 50 times within a consecutive six-month period.
You may be reimbursed up to $200 on gym membership fees if you go to the gym at least 50 times within a consecutive six-month period. In addition, if your spouse may be reimbursed up to $100 for going to the gym at least 50 times within a consecutive six-month period.
The UnitedHealthcare’s Wellness Fitness Reimbursement Program (Fitness Incentive is a participating gym) will reimburse you $20 per month (to an annual maximum of $240) for every month you visit a participating gym at least 12 times. The reimbursement will be deposited directly into your bank account or credited to your fitness center membership dues, depending on the fitness center you select.
For plans that require it, you can easily obtain proof of membership and attendance by directly accessing your Fitness Incentive account online. You’ll be able to see and print your check-in history, your payment history and your membership agreement.
